- 目的 应用数据挖掘技术分析李延萍主任中医师治疗类风湿关节炎的用药规律。方法 选择2018年12月31日-2020年12月31日李延萍主任中医师在重庆市中医院门诊治疗的类风湿关节炎患者,利用中医传承辅助平台V2.5,统计处方涉及药物的使用频次,使用关联规则、聚类分析等数据挖掘方法,得到中药常用药对、药物组合及新方。结果 得到门诊处方101首,涉及中药127味,其中使用频次≥30的有桂枝、甘草、当归、白芍、姜黄、僵蚕、黄芪、薏苡仁、威灵仙、白术、防风、川牛膝,功效多为益气补血、散寒止痛、健脾利湿、活血通络;常用2味药组合87个,频次最高的组合是白芍-桂枝(42次);3味药药物组合29个,频次最高的药物组合为甘草-白芍-桂枝(28次);4味药药物组合1个(21次)。得到由姜黄、僵蚕、薏苡仁、桑寄生、熟地黄等组成的新处方5首。结论 李延萍主任中医师治疗类风湿关节炎用药特点鲜明,药物归经以脾、肝、肺经为主,重视脾胃,以调和营卫、健脾益气补血为基础,并用姜黄、僵蚕、薏苡仁加强除湿止痛、活血通络之效。
- Abstract:
- Objective To analyze the medication law of Li Yanping, chief physician of TCM, in the treatment of rheumatoid arthritis (RA) by using data mining technology. Methods RA patients received by Li Yanping from December 31st, 2018 to December 31st, 2020 at the outpatient department of Chongqing TCM Hospital were chosen. TCM Inheritance Support System V2.5 was used to count the frequency of use of drugs in prescriptions. Data mining methods such as association rules and cluster analysis were used to obtain commonly used TCM pairs, medicinal combinations and new prescriptions. Results 101 outpatient prescriptions were obtained, involving 127 kinds of Chinese materia medica, of which the frequency of use ≥30 were Cinnamomi Ramulus, Glycyrrhizae Radix et Rhizoma, Angelicae Sinensis Radix, Paeoniae Radix Alba, Concisum Rhizoma, Bombyx Batryticatus, Astragali Radix, Coicis Semen, Clematidis Radix et Rhizoma, Atractylodis Macrocephalae Rhizoma, Saposheikovize Radix, and Cyathulae Radix. The efficacy was mainly reinforcing qi and replenishing blood, dissipating cold to kill pain, invigorating the spleen to resolve dampness, promoting blood circulation to remove obstruction from meridians. Totally 87 medicinal pairs were obtained, and the combination with the highest frequency was Paeoniae Radix Alba-Cinnamomi Ramulus; totally 29 medicinal combinations with three drugs were obtained, and the combination with the highest frequency was Glycyrrhizae Radix et Rhizoma-Paeoniae Radix Alba-Cinnamomi Ramulus; one medicinal combination with four drugs was obtained (21 times). Five new prescriptions with Concisum Rhizoma, Bombyx Batryticatus, Coicis Semen, Taxilii Herba, and Rehmannize Radix et Praeparata were obtained. Conclusion With distinctive characteristics of medication, channel tropism of medicine used by Li Yanping, chief physician of TCM, focuses on spleen, liver and lung meridian, and she attaches great importance to spleen and stomach. Based on harmonizing the nutritive and defensive levels, invigorating the spleen, benefiting qi and replenishing blood, she makes good use of Concisum Rhizoma, Bombyx Batryticatus and Coicis Semen, so as to strengthen curative effects of eliminating dampness to relive pain, promoting blood circulation to remove obstruction from meridians.
